Health systems purchase Axial Patienttm in order to engage patients in the management of their care. The application, which is branded by health systems, provides access to tools for tracking health and content from the Mayo Clinic Health Library. Axial Patient’s premium offerings include personal health records, care team communication, and interactive care plans for both acute and chronic recovery.

Axial Exchange is focused on simultaneously lowering avoidable readmissions and improving patient satisfaction scores. Axial has built its technology around proven, evidence-based methods such as Project RED from Boston University Medical Center and Project BOOST from the Society of Hospital Medicine.
