Active Pathways provide a range of high quality recovery focused mental health services for adults aged 18-65 at our two centres; The Hamptons and Brookhaven both situated in Lancashire in the North-west of England near Preston.
Our service ethos is centred on the belief that all our service users should be able to lead an active and fulfilling life. As an organisation we are committed to putting rehabilitation at the heart of everything we do to ensure that our service-users can build a more meaningful and fulfilling life within the community.
We work in partnership with our service users, their families, care coordinators and local commissioners to provide a tailored care package which provides support in their recovery. We aim to improve each individual’s quality of life and create a better future. As an organisation we are committed to using the latest evidence-based practice, and a range of outcome measures to underpin the delivery of our person-centred recovery approach.
In addition to our mental health services, we operate a well established, CQC registered, and highly respected independent home care service; Burrowbeck Community Care in Lancaster. We recognise that our focus must always be on the people we support, their needs, wishes and aspirations. Our domiciliary care service offers a wide range of flexible one-to-one services tailor made to the needs of the individual – and home visits can be from an hour a day up to 24 -7 care, including weekends and holidays.
Active Pathways put in place the holistic care needed to support the full range of a persons physical, mental and social needs. The services we provide are flexible to the needs of each client, these can include personal care such as assistance with bathing, dressing, eating and medication; home help covering all aspects of day-to-day housework, shopping, meal preparation and household duties; and companionship services that can involve everything from escorting clients on visits or appointments to simple conversation and good company.
